Obtaining a Pro-Forma invoice

1. Start to order online using our online shop.
2. Complete the order form and select the "Pro-Forma Invoice - by Check or Wire" payment option
3. Print out the Pro-Forma invoice
4. Follow the on-screen instructions for ordering by "Pro-Forma" to the letter
